Join the Alameda County Care Alliance's 4th Annual Caregiver Recognition Celebration & Health Expo

On Sunday, October 28, PHI's Alameda County Care Alliance (ACCA) will honor and lift up caregivers for their sacrifice and service to help their loved ones at the 4th Annual Caregiver Recognition Celebration & Health Expo. Guests will also learn about the ACCA's Advance Illness Care Program™ and other community resources that may be helpful to them at the Health Expo.
The event will celebrate the success and national recognition of the Alameda County Care Alliance and the Advanced Illness Care Program. Over the last four years, this program has helped over 2,200 persons needing care and caregivers from our congregations and communities. In partnership with health systems, community-based advocacy organizations, and academic organizations, the ACCA has expanded the program to 14 churches in Alameda County and Contra Costa County.
If you are a person needing advance illness care or are a caregiver that needs assistance, the ACCA is here to help. The ACCA Care Navigators work to identify and address needs, provide trusted resources and empower persons needing care and caregivers in 5 big cornerstones (spiritual, planning for advance care, health, social, and caregiving), all free of charge. The ACCA is aware of and sensitive to the emotional, spiritual, and physical demands of caring for others and seeks to assist caregivers in taking care of themselves while caring for others.
Attendees will enjoy an afternoon of activities that include a complimentary celebratory supper, caregiver appreciation service with special performance by the One Mass Choir. Guests will be able to interact with community organizations that can provide assistance with health and advanced illness needs. Caregivers from the community are invited to attend this uplifting event!
